Alternate
Resources Office
The Alternate Resources Office looks for any type of funding i.e.
Medicaid, Medicare, private insurance or any state, federal or local
program that pays for any type of medical services or medical equipment.
It may be important to apply for an alternate funding resource due
to the high cost of medical care. The Alternate Resource coordinator
and assistant work very closely with the State Department of Human
Resources and the Social Security Administration in assisting patients
with eligibility determination for Medicaid and Medicare benefits.
Private insurances are also used.
In some predetermined cases, the Tribal Council can approve
funding to purchase contracted medical services when patients are
not eligible for Medicaid, Medicare or other private insurance.

The Ambulance Service provides for immediate emergency
care and transport to the hospital for treatment or to other hospital
facilities when necessary. Qualified, trained emergency medical technicians
and paramedics are available 24 hours a day, seven days a week. Business

Community Health Services enables members of the Mississippi
Band of Choctaw Indians to achieve optimal health by improving each
individual's health knowledge, attitudes and practices. The responsibilities
of community screening for health problems, health education, prevention
and follow-up of medical care are accomplished through the services
offered at the three primary field clinics. The program provides
health education to patients alone and in groups and in patients
homes. The program maintains major responsibilities for many specific
programs including:
- Audiology (Otitis Media) Screening for hearing problems is provided
by an Audiometric Technician to kindergarten, first grade, third
grade, twelfth grade and teacher and clinic referrals
- Optometry a licensed optometrist gives eye examinations and
makes fittings, adjustments and minor repairs to eye glasses
- Tuberculosis surveillance staff screen for tuberculosis and
follow-up on all persons exposed to assure appropriate care
- Diabetic retinopathy a screening program and a diabetic retinopathy
registry is maintained by the staff. Once every year each person
with diabetes is given an appointment for a diabetic retinopathy
examination.
.Contract
Health Services
The Contract Health Service Area as defined by the Federal government
includes Attala, Winston, Noxubee, Leake, Neshoba, Kemper, Scott,
Newton, Jasper and Jones counties. Limited contract health services
are available to eligible persons for medical care that cannot be
provided at the Choctaw Health Center. Due to limited funds, there
is a priority system in place that is used to determine relative medical
need. The priority system is:
- PRIORITY I Care is necessary immediately to prevent death or
serious threat to life, limb or organ.
- PRIORITY II Care is needed within 30 days to 60 days to prevent
the loss of life, limb or organ. This care is needed to maintain
normal daily functioning.
- PRIORITY III Care can be safely postponed for more than 60 days.
- PRIORITY IV Cosmetic care.
Eligibility: Referred by a Choctaw Health Center doctor to an off-reservation
facility Member of the Mississippi Band of Choctaw Indians Reside
in the Contract Health Service delivery area (defined below) Be a
non-Indian pregnant female residing in the Contract Health Service
delivery area and carrying the child of an eligible Choctaw Indian
Referrals for care that cannot be provided at the Health
Center are prioritized based on this system. Currently, the Center
can only pay for services that are Priority I. If the care is a
priority II-IV, the patient will be responsible for the payment
of the services unless additional funds are made available.

Program
The Dental Clinic is staffed by three dentists and nine
dental auxiliaries. Due to the tremendous number of eligible patients
the clinic must serve, the services available are limited.

.Diabetes
Management Center
Established in the fall of 1999, the Diabetes Management
Center provides comprehensive prevention education and medical treatment
service to every person with diabetes, their families and community
members. The long range goal of the Center is to reduce the incidence
of diabetes and complications of the disease through nutritional,
physical and behavioral (lifestyle) modifications as well as individualized
medical management. Patients with diabetes may be referred from outpatient,
inpatient, Women's Wellness Center, field clinics or by self-referral.

.Dietary
and Nutrition
Dietary and Nutrition services at the CHC provide patients
with a comprehensive nutrition program as a part of total care.
Meals and extra nourishment are prepared and served by a staff of
nine employees. Meals are attractively served, palatable, nutritionally
adequate, therapeutically correct and culturally acceptable. Nutrition
assessments, counseling and education is offered by two registered
dietitions.

Medical Services
Classified for Level IV Trauma Center, the department is
staffed with one physician on call twenty-four hours, RN1 LPN and
a vital signs technician on rotational shift.

Health Program
The major goals of the Environmental Health Program (EHP) are
to reduce and prevent environmentally-related diseases and injuries
and assure the highest level of health, safety and comfort to families
on the Choctaw reservation.

Information
The Health Information Department provides assistance
to health care workers and patients by maintaining a progressive health
record system that encompasses all aspects of a comprehensive health
program. The department maintains readily available, complete and
accurate health records in all medico-legal aspects, maintains strict
regulations regarding confidentiality and protects patients rights
to privacy at all times.

.Inpatient
Department
Choctaw Health Center provides an inpatient unit for individuals
in need of observation, acute medical care, intense teaching or continuation
of prescribed treatment. This unit consists of adult rooms, pediatric
cribs/bassinets, three private rooms, two private rooms for isolation,
nine two-bed rooms and one four-bed unit.
Our inpatient staff consist of RNS, LPNS, Nursing Assistance,
and a unit clerk to provide continunity of care.
.Lab
Services
A full range of tests are performed at CHC. Tests not
analyzed on site are sent to Medical Pathology Laboratories in Meridian.
The laboratory is open for routine and emergency services.
Weekends are contracted for a 48-hour time period during which the
lab person on duty is required to be available as needed. The lab
has six full-time employees.

Department
The goal of the Outpatient Department is to provide readily
available, comprehensive, high quality ambulatory health care to the
members of the Choctaw community and other eligible persons. Referrals
for specialty contract health services are available to those who
meet the eligibility criteria. All services are free to eligible persons.
However, insurance companies, Medicaid and Medicare are billed for
services at this facility whenever possible.
Patients are encouraged to make an appointment unless it
is an emergency. Without an appointment, the patient must wait until
the scheduled appointments are seen first.

.Pharmacy
Service
Pharmacy Services coordinates pharmacy activities throughout
the facility and provides direct and indirect pharmacy coverage
and supervision to patient care departments. The service consists
of a chief pharmacist, four staff pharmacists, one pharmacy technician,
a pharmacy assistant and a pharmacy clerk. Pharmacy Services covers
both inpatient and outpatient services and conforms to both Federal
and State laws. Pharmacy Services works closely with the medical
staff, nursing, health information, dietary, diabetic education,
community health, Women's Wellness Center and other departments
when necessary.

.Radiology
Department
Diagnostic radiology, mammography and ultrasound services are
available to eligible patients through referring physician and nurse
practitioner requests. These services are used to monitor the health
condition of the individual patient and assist the medical staff
in reaching a diagnosis. Routine radiologic examinations (bone and
soft tissue) do not require scheduling and can be ordered at the
physician's discretion. Fluoroscopic, tomographic and ultrasound
exams require scheduling due to the patient preparation involved.
Mammography also requires scheduling.

.Transportation
Department
Regular transportation service is available only for patients with
appointments at CHC. It is not available on weekends or holidays unless
requested by the physician on call or the nursing supervisor. The
purpose of the transportation department is to provide non-emergency
transportation for patients to the CHC or contract health facilities
in distant cities. Priority is given to individuals who have absolutely
no method of transportation or are physically incapable of operating
a vehicle.

.WIC
(Women, Infants, & Children)
The Special Supplemental Nutrition Program for Women, Infants and
Children (WIC) is a United States Department of Agriculture (USDA)
funded program. Its emphasis is on nutrition education primarily targeting
low-income families with pregnant, breast-feeding and postpartum women;
infants and children up to age five and at nutritional risk.
Other
benefits include referrals for other health/social services and
a food prescription. The food prescription is a set of selected
foods in specific quantities appropriate for the individual based
on nutritional risk. These prescriptions are redeemable at authorized
retail grocers. Food prescriptions for infants are redeemed at the
WIC Office, by direct distribution of infant formulas, infant cereals
and juices. Food Prescription (voucher) Issuance Schedule WIC staff
go to each community for issuance of food prescriptions every other
month and issue two months worth of prescriptions. Participants
can check with the WIC Office to determine the date, location and
time of issuance in their community.
Coordination with Other Services
While WIC is not a direct service sponsored by the Choctaw Health
Department, the WIC Office is conveniently located in a trailer
next to the Women's Wellness Center. The two programs coordinate
for the convenience of clients who need the services of both. Other
health services and programs are also within a short walking distance.

Eligibility
All applicants must meet three criteria in order to be certified
eligible for WIC:
- All applicants must pass an income eligibility determination.
Individuals at or below the 185 percent of the Federal Income
for Poverty Guidelines are eligible. Income documentation will
be required. Applicants need to bring check stubs for all family
members earning income in the household. Applicants providing
proof of participation on Medicaid, Food Stamps and Temporary
Assistance for Needy Families (TANF) are adjunctively eligible.
Those providing proof as participants on FDPRI (Tribal Food Distribution
Program) or receive SSI are automatically income eligible.
- Being income eligible is not enough to be certified for WIC.
The applicant must also be at nutritional risk. Getting a blood
sample, weight and height, and a diet assessment from the applicant
helps make a determination of nutritional risk. Current or existing
medical condition(s), such as gestational diabetes, failure to
thrive, anemia, etc., that is impacted by diet will be factors
in determining eligibility.

- Tribal ID card or a corrected CHC Patient Registration F.A.C.E.
Sheet.
.WIC
Farmer's Market Nutrition Program
Certified WIC Program participants are eligible to participate
in the WIC Farmers' Market Nutrition Program (FMNP), funded by the
U.S. Department of Agriculture (USDA). The Choctaw Farmers' Market
has been authorized to provide fresh produce.
The location of the market is on Black Jack Road, off Hwy 16 West,
in the Pearl River Community.
The market season is generally June through October. Date and
hours of market operation will be advertised prior to its opening.
WIC FMNP participants will recieve:
- Fresh produce. An approved list of fruits, vegetables and herbs
is available.
- Nutrition Education on topics that surround fresh fruits, vegetables
& herbs. During the market season, classes will be held at the
market.
Access for those participants, who do not have reliable transportation
and cannot access the market on Black Jack road, they can receive
benefits by going to their local vacility building. A date and time
is scheduled for their community and staff from the market will
set up a "mobile market" in outlying communities.
Supplemental Income for Local Farmers
Many tribal farmers, who do some home gardening or small-scale
farming, benefit by participating in the program. The market is
open to all bona fide farmers who can deliver quality produce in
sufficient quantities grown locally. The Choctaw Farmer's Market
defines "locally grown" as produce grown in the state of Mississippi,
more particularly within the counties in which Tribal land are located.

Wellness Center
The Women's Wellness Center focuses on providing comprehensive
preventive health care and health education for all Choctaw women
and children (newborn through five years of age). A team approach
is followed for each client. Each client's health education plan
is based upon the individual's needs.
